No Surprise Act Policy
The office of Functional Health of O’Fallon will review the office appointment pricing verbally, by phone and/or in-person, when a new patient calls the office to schedule their appointment/s. We are an appointment time-based clinic. All treatment is included in the patient’s appointment time. This treatment includes the following: chiropractic, muscle work, acupuncture, lab/record/imaging review, NeuroEmotional Technique, nutritional information, Applied Kinesiology, Quantum Neurology, and patient education. Any additional lab/imaging orders, nutritional supplements and/or decompression treatments recommended by Dr. Daniels are NOT included in the pricing below. The office staff will review the pricing with the patient PRIOR to purchasing nutritional supplements, decompression treatments and lab/imaging orders.


 
Privacy and Sharing of Information Policy
The office of Functional Health of O’Fallon and its staff will collect personal and medical information for patients. All patient’s personal and medical information is confidential and will only be disclosed to third parties with the patient’s permission.
The office follows HIPAA- Privacy Policy- General Rule (164.502).
All patients have the right to a copy of their personal health information (PHI)/records, electronically and/or printed. Please contact the front desk for a copy of your records, if needed. Only the patient, the patient’s guardian (if a minor), the patient’s emergency contact or if a release of records is signed by the patient, is when the office will release the patient’s PHI. The office will have the patient’s records completed and to the patient as soon as possible. Our goal is to have record requests completed 2 weeks of receiving the request. This request can be made verbally, in-person or by release of records.
Dr. Daniels may deny access to a patient’s PHI when Dr. Daniels reasonably believes not providing the records is reasonable and necessary to prevent harm to a patient.
Patient electronic health records are protected and if a breach of PHI occurs, we will contact the patient immediately.
